Become a SILCC Host
Hospital pharmacies willing to be part of the SILCC Programme will need to fill out and send the SILCC Host application form to Statements@eahp.eu. All SILCC Hosts need to assess their pharmacies using the EAHP self-assessment tool.
In addition, applicants need to send the following supporting documents:
- PDF Copy of your Assessment with the level of Implementation for all the Statements
- PDF Copy of the overall score

Become a SILCC Fellow
Hospital Pharmacits willing to become a SILCC Fellow will need to fill out and send the application form to Statements@eahp.eu. All SILCC Fellows need to assess their hospital pharmacies using the EAHP Self-assessment tool.
In addiiton, applicatns need to send the following supporting documents:
- PDF Copy of your Assessment with the level of Implementation for all the Statements
- PDF Copy of the overall score
- PDF Copy of your Action Plan
